We appreciate your interest in becoming a General Member of The Cooperative!

As the state-licensed Maternal and Child Health Consortium (MCHC) for the seven counties of South Jersey, The Cooperative is responsible for managing perinatal and pediatric health care services for this region. As such, the Cooperative provides an excellent opportunity for those who are concerned about the design, development, and delivery of perinatal and pediatric health care services in South Jersey.

Application Process

Applications for membership are reviewed by the Membership & Nominating Committee, which makes a recommendation for approval to the Board of Directors. The Board meets regularly and action on membership applications is a standing agenda item.

In addition to completing the application, All applicants for General Membership are asked to review the agency’s bylaws and sign the “Bylaws Subscription” statement.

Please note that applications are only approved by the Board of Directors at their meetings on a quarterly basis (January, March, June, November).
Membership is limited to individuals living or working in South Jersey. After completing this online form, you will receive additional application materials via mail.​

The Cooperative has been serving this region for more than four decades and we are proud of our long list of accomplishments. Clearly, much of The Cooperative's success is attributable to the many remarkable individuals who have given generously of their time and expertise.
